No need for initiative during this portion.  None of them are within view at the moment.  And yes both the doctor and the free clinic are references to Dr. Mid-Nite.

Heh.  Alrighty.  Graft is able to catch notice of a muzzle poking out from a nearby hall intersection.  Feel Free to take whatever action he deems necessary in response.  The minion holding said rifle would of course not see him until he was right in front of it so he'll be unaware to any action coming his way.

Same situation as per the earlier minion in that they're completely unaware to the heroes' pressences.  But are not close enough together to validate describing a takedown attack.  That all in mind since these guys will be taken out narratively still at this point just consider any fancy stuff like destroying the light fixture (or just flipping the switch) all within the same motion.
